Indications

Paracetamol is indicated for fever, cold and flu, headache, toothache, earache, muscle pain, muscle pain, neuralgia, menstrual pain, sprain, abdominal pain, backache, backache. surgery, postpartum pain, inflammatory pain and post-vaccination pain in children. It is also indicated for rheumatic and inflammatory conditions and stiffness.

 

Pharmacology

Paracetamol has analgesic and antipyretic properties with low anti-inflammatory activity. Paracetamol (acetaminophen) is thought to have a major CNS effect, increasing pain threshold by inhibiting two isoforms of cyclooxygenase, the enzymes COX1, COX2, and COX3 involved in prostaglandin (PG) synthesis. . Paracetamol is a derivative of para aminophenol, which has analgesic and antipyretic properties with low anti-inflammatory activity. Paracetamol is one of the most widely used, safe and fast pain relievers. It is well tolerated and free of the various side effects of aspirin.

 

Dosage & Administration

Tablets:

  • Adults: 12 tablets every 4 to 6 hours up to a maximum of 4 g (8 tablets) per day.
  • Children (612 years old): ½ to 1 tablet 3 to 4 times a day. For long-term treatment, an overdose in excess of 2.6 g/day is not recommended.

 Extended-release tablets:

  • Adults and children over 12 years of age: Two tablets, to be swallowed whole, every 6 to 8 hours (maximum 6 tablets every 24 hours). Tablets must not be crushed.

 Syrup / Suspension:

  • Children under 3 months: 10 mg/kg body weight (reduced to 5 mg/kg in case of jaundice) 3 to 4 times a day.
  • 3 months to less than 1 year: ½ to 1 teaspoon 3 to 4 times a day.
  • 1-5 years: 1 2 full tablespoons 3 to 4 times a day.
  • 6-12 years: 2A teaspoon 3 to 4 times a day.
  • Adults: 48 full tablespoons 3 to 4 times a day.

 Suppositories:

  • Children from 3-12 months: 60 120 mg, 4 times a day.
  • Children aged 1-5: 125 250 mg x 4 times / day.
  • Children 6-12 years old: 250 500 mg x 4 times / day.
  • Adults & children over 12 years:51 g x 4 times / day.

 Drops for children:

  • Children up to 3 months: 5 ml (40 mg)
  • 4 to 11 months:0 ml (80 mg)
  • 7 to 2 years:5 ml (120 mg). Do not take more than 5 doses per day for a maximum of 5 days.

Paracetamol tablets with Actizorb technology: it dissolves 5 times faster than standard paracetamol tablets. It is a safe, fast-acting pain reliever with pronounced antipyretic properties. It is particularly suitable for patients who, for whatever reason, cannot tolerate aspirin or other pain relievers.

  • Adults and children (12 years and over): Take 1 to 2 tablets every four to six hours as needed. Do not take more than 8 tablets in 24 hours.
  • Children (7 to 11 years old): Take ½ 1 tablet every four to six hours as needed. Do not take more than 4 tablets in 24 hours. Should not be used in children under 7 years old.

 

Interaction

Patients who have taken barbiturates, tricyclic antidepressants and alcohol may have a reduced ability to metabolize large doses of paracetamol. Alcohol can increase liver toxicity from paracetamol overdose. Chronic administration of anticonvulsants or oral steroid contraceptives induces liver enzymes and may prevent reaching therapeutic levels of paracetamol by increasing first-pass metabolism or clearance.

 

Contraindications

It is contraindicated in known hypersensitivity to Paracetamol.

 

Side Effects

Side effects of paracetamol are generally mild, although haematological reactions including thrombocytopenia, leukopenia, thrombocytopenia, leukopenia and agranulocytosis have been reported. Sometimes pancreatitis, rashes, and other allergic reactions occur.

 

Pregnancy & Lactation

Pregnancy category B according to USFDA. This drug should be used during pregnancy only if clearly needed

 

Precautions & Warnings

Care should be taken when administering paracetamol to patients with impaired hepatic or renal function. Care should be taken when administering paracetamol to patients taking other drugs that affect the liver.
