Generic

Metronidazole + Miconazole Nitrate

 

Indications

The vaginal treatment of female genitourinary trichomoniasis infection, the prevention and local treatment of candidiasis.

 

Pharmacology

Metronidazole is converted into a reduction product, interacts with DNA, destroys the DNA helical chain and structure, and inhibits protein synthesis and cell death of susceptible organisms. It is active against most anaerobic protozoa, some gm + ve, gmve and facultative anaerobes.

Miconazole inhibits the synthesis of ergosterol, thereby destroying the cell wall membrane of the fungus and increasing its permeability, resulting in leakage of nutrients.

 

Dosage & Administration

In trichomoniasis, concurrently with oral metronidazole treatment and 1 vaginal tablet should be inserted high up into the vagina once a day (preferably before going to bed at night) for 10 days. Long-term recovery may be expected only is response to the simultaneous oral treatment of both partners.

In candidiasis or other fungal infections, 1 vaginal tablet has to be inserted high up into the vagina for 10 days. The vaginal tablet should be slightly moistened before application.

 

Interactions

Metronidazole: reaction similar to disulfiram with alcohol. Oral anticoagulation, phenytoin blood concentration, lithium intoxication, astemizole and terfenadine plasma cone increased; the blood cone of carbamazepine. Use phenobarb to lower blood levels. Disulfiram causes the risk of effects related to the central nervous system (such as psychotic reactions). The use of cimetidine increases blood levels and neurological effects. May increase blood concentration and toxicity of fluorouracil; cyclosporine toxicity. Cardiotoxicity increases when combined with amiodarone. Interference with blood levels of liver enzymes, glucose (hexokinase method), theophylline, and procainamide. The level of phenytoin sodium decreases.

Miconazole: Acenocoumarin, anisidinedione, dicoumarin, phenindione, phenprocoumarin, and warfarin increase the risk of bleeding; exposure to plasma cones and oxybutynin. 
may increase the toxicity risk of phenytoin, fosphenytoin, cyclosporine, and trimetrexate, and the cardiotoxicity of pimozide. May inhibit the metabolism of astemizole, cisapride and terfenadine. Reduce the metabolism of carbamazepine. The effect of opioids (fentanyl) is increased or prolonged. Combination with glimepiride may cause hypoglycemia. The clearance rate decreases and the plasma oxycodone cone increases. May increase the bioavailability of tolterodine (in patients with insufficient CYP2D6 activity).

 

Contraindications

Hypersensitivity; hepatic impairment (oral gel). Porphyria.

 

Side Effects

Nausea, vomiting, febrile reactions, rash, drowsiness, diarrhoea, anorexia and flushing, hepatitis. Local irritation and sensitisation, contact dermatitis.

 

Pregnancy

Animal reproduction studies have failed to prove the risk to the fetus, and there are no adequate and well-controlled studies on pregnant women, or any three-month risk to the fetus. 

Animal reproduction studies have shown adverse effects on the fetus, and there are no adequate and well-controlled studies in humans, but the potential benefits may justify the use of the drug in pregnant women despite the potential risks.
